Employer Information

Employers Full Name
Enter the first and last name of the employer, or the representative of the employer.
Business Address
If you have not received an EIN, contact us for assistance. An additional set-up fee of $50 will apply. If you don't have an EIN, leave blank, we will update your file when received.
If you have not received a State ID, contact us for assistance. An additional set-up fee of $150 will apply. If you don't have a State ID, leave blank, we will update your file when received.

Banking Information

Enter the banking information you would like to use for payroll taxes and payroll wages.
Enter the name of the bank or credit union where employer funds will be debited (Bank of America, Navy Federal Credit Union, Chase, etc.).
